It is estimated that 95% of hair loss amongst men is caused by male pattern baldness. This occurs when an enzyme known as 5-alpha-reductase interacts with the testosterone levels in the skin of the scalp to produce a new hormone called Dihydrotestosterone (DHT). Some hair follicles react to the presence of DHT in the skin and start to shrink. This inhibits the natural growth cycle of the hair and can cause the follicle to ‘shut down’. Old hair is then no longer replaced by new growth and the signs of male pattern baldness begin to show. The only way to prevent this from happening is to block the creation of the DHT hormone, which is effectively the action of Propecia.

In a series of clinical trials over a five year period involving men with mild to moderate hair loss, 48% of those treated with Propecia (finasteride 1 mg) experienced some regrowth of hair, and a further 42% had no further loss. Average hair count in the treatment group remained above baseline and showed an increasing difference from hair count in the placebo group. Propecia is effective only for as long as it is taken; the hair gained or maintained is lost within 6–12 months of ceasing therapy. In clinical studies, Propecia, like Minoxidil, was shown to work on both the crown area and the hairline, but is most successful in the crown area.

Propecia has been shown to maintain or increase the amount of hair in 83% of men who used the product and has promoted visible hair growth in 66% of men. It has also been clinically shown to reduce the amount of DHT in the scalp by 60%.

More recent studies have shown that Propecia is also successful in reversing frontal hair loss, although slightly less effective than at the vertex of the scalp. Whereas Minoxidil (as the branded product Regaine) is a lotion that can be messy and awkward to use, Propecia is simply taken in tablet form once a day.

Most men start to see visible results within three months with a decrease in the amount of hair lost and a general improvement in the condition of the hair. Regrowth is noticeable after six months with some men regaining almost all the hair they had previously lost.
